SummerScope 2012 Enrollment Information
Exiting 1st Grade - 6th Grade for the 2011/12 school year
Start: May 30, 2012 End: Aug. 15, 2012 11 weeks of programming.

Early Registration
Early registration (for those who were in the 2011 program only) for the 2012 program will begin August 1st, 2011. This will be the actual registration, not pre-registration. WE MUST RECEIVE BOTH THE ENROLLMENT FORM AND REGISTRATION FEE AT THE SAME TIME! Both are required to register your child. It will be up to you to turn the forms and money in by August 12th, 2011. There will be no other early registration for those who were in the 2010 program until February 1st, 2012. You will be asked to confirm enrollment starting in Feb. 2012.
FBC Member Registration
Any family who has a child currently enrolled in any of the children’s programs has an early registration time. Beginning 8:30am on Feb. 1st, 2012 you may come by the office with all the enrollment information and registration fee to enroll your child/ren.
Public Registration
Open registration to the public begins March 1st, 2012. First come first serve to any remaining positions. Doors open at 8:30am.
Required forms include (but are not limited to):
• Tuition Contract
• Medical Release Form
• Child Information Sheet provided by Oklahoma DHS
• Swim/Transportation Permission
• Updated Shot Records
All forms must be completed before the first day of attendance!
Registration Fee
The registration fee will be $50 for one child or $75 for more than one. This registration fee is non-refundable and is not included in the tuition.
Tuition Contract
A written contract stating amount due, dates due, and consequences for non-payment must be signed and dated by both the patron and the Executive Director. The patron will receive a copy of the signed document before registration is completed. Any financial arrangements must be agreed upon by the Executive Director or the Minister to Children.
Part-Time vs. Full-Time
Part time: 3 days or less a week = 33 days for the whole summer
Full time: 4 days or more a week
Any change in status from part-time to full-time or from full-time to part-time will require permission from the Summerscope Director. After June 15th, there will not be any change from full-time to part-time status unless there are extenuating circumstances.
